Client ID for Air Traffic Services: FL0345
name- This must be your actual name, not your business name. If you have other employees that you would like to have covered, you would need to enroll them separately.
Social Security Number or Tax ID- Even though NICA requires you to enroll under your name, if the want the income to be under your business, use your Tax ID.

primary phone number- This must be a landline in your name. If you do not have one, try a mobile number in your name. If you don’t have either of those in your name, you can try your phone number but it might not accept it or might ask you questions that are meant for the owner of that number.
what city you were born in
mother’s maiden name
financial info- You may want to have your financial info close to you so you can answer the identity check questions. Typical questions include:
- Who do you pay your mortgage or car payment to?
- How much is the above payment?
- What street, city or county have you lived on/in in the past?
- What country was your driver’s license issued in?
- What state was you social security number issued in?
DOT number- If you have a vehicle greater than 18 feet, you will need to know your DOT number.
Type of business- If you have a business then you will need to know what type. Chances are, if you have one, you know what type it is. If you do not, you will choose “sole proprietor” on that section.
